Summary:
Reporting to the Chief Nursing Officer (CNO), the Mental Health Inpatient Services Liaison is responsible for assessing, planning, coordinating and implementing services for individuals with mental illness; includes the provision of continuity of care services at the Center's contracted Community Mental Health Hospital as well as at other community and state hospital facilities as identified. Continuity of care services including, but not limited to, prescreening for commitment hearings, attendance & participation in hearings as scheduled, assists with facilitation of inpatient admissions, participates in clinical rounds led by UTMB attending physicians, and responsible for aftercare/discharge planning & referrals. Regularly consults and interfaces with Gulf Coast Center clinical/support staff, mental health deputies, judiciary, criminal justice facilities and others as appropriate. Also responsible for ensuring quarterly & semi-annual achievement of identified HHSC performance contract measures/outcomes.

The Case Manager II - Multi-Service Liaison will be responsible for the following:
Essential
Job Duties:
- Participates in continuity of care activities associated with inpatient service delivery; attends daily rounds with UTMB physicians, interfaces with hospital clinicians/support staff, MH Deputies, judiciary and GCC program staff so as to ensure that all COC services and program requirements are being met pursuant to GCC Policies & Procedures and the HHSC performance contract.
- Will provide prescreening activities for individuals seeking voluntary admission or identified as needing inpatient treatment or commitment; assesses identified individuals, submits ATR to courts, attends weekly commitment hearings as scheduled (Mainland Medical Center/St. Joseph), provides recommendations to the court and advocates for least restrictive care for the individual.
- Maintains a current knowledge base with regard to rules, regulations, policies and procedures relating to behavioral health inpatient service delivery; assists the MH Lead Liaison with the development guidelines and COC policies & procedures; in partnership with other identified MH Liaisons, & under the direction of HR & Chief Nursing Officer, will assist with the development, review and revision of the COC Policies & Procedures annually and/or as needed.
- Participates in inter and intra-agency communications to share information regarding quality of care, inpatient recidivism, challenges as they relate to inpatient services; works across GCC service delivery areas to achieve objectives; participates in GCC COC recurring meetings as well as other inter-departmental planning meetings to address readmission rates, length of stay rates and co-occurring substance use disorders; regularly communicates with GCC UM Manager regarding authorization for inpatient services.
- Utilizing LCDC training, provides outreach, screening, assessment, and referral (OSAR) to individuals hospitalized at contracted inpatient unit with documented chronic substance use disorder in need of substance use treatment services; possess knowledge of local community resources & network with agencies to ensure; conducts screenings on identified days of the week; documents all interactions/screenings in CMBHS; coordinates & arranges referral to appropriate residential/rehab facility upon inpatient discharge; outside residential facilities or GCC subcontracted facilities; coordinates with OSAR placement on wait list if necessary; coordinates referral for outpatient (IOP) where clinically appropriate; coordinates with OSAR/MCOT identified staff for warm hand-off where appropriate.
- Keeps supervisor informed by reviewing and analyzing daily census reports, SmartCare & CARE bed day reports, readmission rates, summarizing information & identifying high utilizers of inpatient services; provides accurate reports to CNO as directed; pursuant to the HHSC performance contract, enters into SmartCare the bed day data on a daily basis so as to ensure accurately and timely reporting in CARE; responsible for reconciliation of SmartCare bed day data & CARE reports relative to inpatient service delivery & monthly achievement of 90% occupancy rate performance measure at CMHH.
- Responsible for aftercare/discharge planning associated with each inpatient admission to Regional or State Hospital in preparation for individuals' integration back to community; with input from individual and inpatient treatment team, develops discharge plan & performs face-to-face aftercare within 7 days from discharge; contingent upon need for community resources, completes/submits the necessary referrals to Crisis Respite services, COPSD, housing & employment and/or other referrals as clinically appropriate; prescriptions for medication are provided to individual as per inpatient medication provision protocols.
- As per HHSC requirement, completes/submits GCC daily census report to HHSC by 10:00am each day; responsible for participating in recurring, monthly HHSC Hospital TA calls in the absence of the MHL Lead as evidenced by electronically submitted report and attendance roster for the HHSC Technical Assistance calls.
- In the absence of the MH Lead, maintains contact with Austin State Hospital (and other SMHFs) for COC and follow-up purposes for individuals receiving long-term inpatient care; participates in conference calls as scheduled, coordinates with physicians & social workers for follow-up & discharge planning to include NGRI identified individuals.
- Ensures that all documentation is complete, accurate and clear as required by GCC clinical documentation guidelines, MH Continuity of Care services procedures, HHSC contract & TAC requirements; documentation is completed & service encounters are entered into SmartCare EHR within 2 business days of service provision as appropriate to patients' treatment plan & assignment in CARE; responsible for entering admission/discharge information daily before 5:00pm; clinical documentation of communication efforts with SMHFs is expected; through a system of care, helps to establish baseline data & assists with tracking vital health indicators as a part of BHCQMs & 1115 Waiver outcome measures.
- Conducts the Adult Needs & Strengths Assessment (ANSA) as clinically indicated for individuals as required & assigned; responsible for ensuring completion of initial & annual ANSA recertification pursuant to the Praed Foundation & HHSC performance contract; a lapse/expiration in certification will result in termination of employment. Exception: If certification lapse/expiration is reported to supervisor within 24 hours of expiration date, employee will be granted opportunity to recertify by close of following business day. Any lapse/expiration greater than 24 hours will result in termination of employment. Additionally, all certified practitioners are responsible for participating in semi-annual quality assurance training activity conducted by the identified ANSA/CANS Super User; maintains current ANSA certification.

Preferred Experience:
