Job Title: Case Manager, QMHP Assisted Outpatient Treatment Program (AOT)

FLSA Status: Non-Exempt

Working Status: Full Time with Benefits

Summary:

The Case Manager- Assisted Outpatient Treatment Program (AOT) is responsible for assisting the staff and clients of the Gulf Coast Center within the Gulf Coast Region Assisted Outpatient Treatment (GCR AOT). Additionally, they are responsible for implementing a multi-disciplinary AOT program, working with courts, community partners, and other entities to support community based treatment for adults with SMI who meet criteria for AOT.

Job Duties: (To be performed with or without accommodation as based on ADA requirements)

The Case Manager, QMHP - Assisted Outpatient Treatment Program (AOT)will be responsible for the following:

Essential Job Duties: 

  • Delivers services in accordance with organizational service delivery standards, maintaining a minimum of 50% of time worked in direct service provision. A graduated ramp-up is expected for new hires: 30% in the first month, 40% in the second month, and 50% by the third month, to support quality, consistency, and client-centered outcomes.

  • Completes accurate, collaborative documentation that is entered and electronically signed within 24 business hours in accordance with regulatory and organizational expectations.
  • Responsible for assisting with implementing a multi-disciplinary AOT program, working with courts, community partners, and other entities to support community-based treatment for adults with SMI who meet criteria for AOT. 
  • Manages collaboration between core partners, identifying and enrolling participants, and developing recovery plans.
  • Primary responsibility will be coordinating and implementing evidence-based educational and support services for AOT program participants as part of an interdisciplinary team.
  • This model incorporates resiliency and recovery-oriented services, supports, practices and beliefs into publicly funded mental health delivery systems.
  •  As part of an interdisciplinary team, and within the scope of the team, will be responsible for consultation with other team members on case management issues; developing full range of options in the community settings; rehabilitation self-management and wellness strategies; coordinating medical documentation within medication checkups for GCC consumers, and technical assistance as part of the team in an individual and/or group capacity.
  • Will provide psychosocial rehabilitation service provision within the TRR service model, including crisis intervention services, pharmacological management services, targeted case management, medication training & support, skills training & development, psychosocial rehabilitative services as well as SE/SH/IMR evidence-based practices within MH adult service array
  • Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position

  • Interact with staff, consumers, local entities and advocacy groups to provide a service that meets local needs.
  • Provide highest level of Customer Service for both internal and external customers.
  • Required to report personnel leave; flex time and emergency leave to assigned supervisor.

  • Consistent adherence to the Center's Dress Code.
  • Consistent adherence to Center safety guidelines and expectations.
  • May be required to travel within Brazoria and Galveston Counties.
  • Out of region travel may be required to attend trainings.
  • Must maintain a clear driving record and driver's license in accordance with our Driver Policy.
  • Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.

Supervision:

  • Receives direct supervision from Project Director- AOT

Required Education:

  • Requires a baccalaureate degree or equivalent in the behavioral sciences field of study from an accredited college or university and/or specialized QMHP certification.

Required Experience:

  • Manager should have general knowledge of mental health disorders, be able to establish cooperative relationships with individuals seeking services, professionals, and organizations, and skilled at working within a team.

Working Conditions:

  • Work will be performed in the Office, Field and/or Remotely setting(s).
  • May require varied work hours depending on need of clients.
  • Out of region travel may be required to attend trainings.
  • May require travel to various locations within Brazoria and Galveston Counties.
  • Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

Special Projects:

  • Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
