Job Title: Community Outreach and Diversion Case Manager

Location: Brazoria County, TX

FLSA Status: Non-Exempt

Working Status: Full time w/benefits

Summary: This is a one year grant-funded position. With an anticipated end date of September 2025

As an integral partner of Coordinated Entry system, HOME- ARP has been afforded the opportunity to develop a Diversion program for individuals and families with minor children In Brazoria County. The Community Outreach and Diversion Case Manager will engage in housing-focused problem-solving by reducing the length of stay in area shelters or avoiding shelter stay altogether. This position will also collaborate with other team members to ensure that essential services are provided to clients accessing services through GCC Diversion Project to increase access to housing stability. The ideal candidate is someone who is able to think strategically and empathetically, function well independently and as part of a team, is highly organized, and can multi-task to achieve desired outcomes.

Job Duties: (To be performed with or without accommodation as based on ADA requirements)

The Community Outreach and Diversion Case Manager will be responsible for the following essential Job Duties:

· Conduct assessments and interviews to determine eligibility for the program prior to program entry.

· Complete intakes and in depth assessment at program entry.

· Developing and updating service plans, case notes and other documents related to a participants file and in accordance with grant requirements and within Center policy for documentation expectations and timeliness.

· Coordinate care and supports for clients and advocate for client's needs. Providing ongoing life skills development including problem solving and skill development to aid in the participants' process of improving independence and quality of life.

· Provide outreach services like Coordinated Entry assessments and community education on the program Collects and maintains accurate resource spreadsheet and/or database Assist prospective individual navigate GCC Diversion Project eligibility process

· Provide short term case management to diversion clients enrolling and exiting the program

· Establishing a broad base of knowledge of community services and communicating appropriate referrals to clients. Engaging with community partners to provide services onsite and offsite

· Conducts these home and community based visits on a regular basis and maintain regular communication with program participants.

· Effectively guide participants through the process of application, qualification, re-determination for SSI, SSDI, Medicaid, Medicare, SNAP, and other services as needed.

· Keep and maintain accurate and up to date client files. This includes data in HMIS and client's physical file.

· Demonstrates a competent, professional, and client-centered approach to services and role model these skills for clients in program services.

· Working knowledge of PSH program goals, structure, and responsibilities and work to obtain clarification from program leadership when needed.

· Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position.

· Treat all individuals with the utmost courtesy, decency and respect.

· Provide responsive, and efficient services.

· Interact with staff, consumers, local entities and advocacy groups to provide a service that meets local needs.

· Provide highest level of Customer Service for both internal and external customers.

· Required to report personnel leave; flex time and emergency leave to assigned supervisor. Strict adherence to the professional, ethical, and legal requirements set forth in GCC's Business Code of Conduct as well as the Corporate Compliance Plan.

· Consistent adherence to the Center's Dress Code.

· Consistent adherence to Center safety guidelines and expectations.

· May be required to travel within Brazoria and Galveston Counties. Out of region travel may be required to attend trainings.

· Must maintain a clear driving record and driver's license in accordance with our Driver Policy. Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.

Non-Essential Job Duties:

· Assist in development of program informational materials

· Attend resource meetings regularly, and maintain accurate resource database

· Cross train with team members to provide essential services necessary to further housing stability needs when necessary

Required Education:

· High School degree, GED or equivalent

Preferred Education:

· Associate's degree in related field with 2+ year experience or Bachelor's degree in Social Work or related field

Required Experience:

· Experience in shelters or with homeless programs designed for adults with serious mental illness and/or mental illness and substance use disorders; knowledge of community based mental health programs (must be verifiable on resume in order to move forward)

Preferred Experience:

· Experience working with individuals with intensive needs such as elderly or history of homelessness; experience working as a case manager with similar/same population or HUD program. Field based experience.

Working conditions:

· Work will be performed in the Field and remote settings.

· May require varied work hours depending on need of clients.

· Out of region travel may be required to attend trainings.

· May require travel to various locations within Brazoria and Galveston Counties. Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

Special Projects:

· Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
