Job Title: Population Health Specialist (Data Analyst) - Co-op

Location: Remote serving Brazoria and Galveston Counties
FLSA Status: Non-Exempt
Working Status: Paid Co-op

Summary:

This position is responsible for ensuring data accuracy, disseminating reports throughout the organization, and managing population health tools. The analyst will analyze clinical data to improve quality and cost in population health and risk-oriented behavioral healthcare operations. Responsibilities include data interpretation and identifying appropriate strategies and interventions, with a focus on quality improvement and evaluation of grant projects in the behavioral health care space. The Population Health Specialist will assist in organization-wide efforts regarding data management and reporting.

Job Duties: (To be performed with or without accommodation as based on ADA requirements)

The Population Health Specialist will be responsible for the following:

Essential Job Duties:

  • Coordinate with the CCBHC Director regarding data reporting, including data pulls from queries specific to organizational operations, grant requirements, and special projects.
  • Develop reports to assist programs, grants, the Project Management Office, and committees in Continuous Quality Improvement (CQI) processes.
  • Ensure required data is collected in a timely manner, regularly report on progress, and provide accurate recommendations for future improvements based on the data.
  • Perform ad-hoc reports as needed.
  • Collaborate with the Business Technology and Transformation department to ensure data integrity.
  • Conduct chart reviews to extract data for necessary reporting.
  • Work with various data reporting and population health tools.
  • Filter, clean, and validate reports by reviewing data and performance indicators to locate and correct reporting problems.
  • Extract and analyze data to support the organization's decision-making processes.
  • Provide recommendations to the QI Director based on professional expertise, data analytics, and insights regarding the improvement of care and organizational systems.
  • Assist the CCBHC Director with meetings, planning sessions, and visualization tools.
  • Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position.

  • Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.

Supervision:

  • Receives direct supervision from the Portfolio Manager and CCBHC Director as part of the PMO.

Required Education:

  • Currently pursuing a Bachelor's or Master's degree in Population Health, Public Health, Informatics, Information Technology, Data Science, or a related degree program.

Preferred Education/Licensure/Certifications:

  • Master's Degree preferred (in Population Health, Public Health, or related field).

Required Licensure/Certifications:

Required Experience:

  • N/A

Preferred Experience:

  • Experience in an operations role in a healthcare setting preferred.
  • 2 years of quality, clinical, or medical record experience preferred.
  • Direct care experience preferred.
  • Electronic medical record experience preferred.
  • Familiarity with CMBHS and SmartCare.
  • Minimum of 1 year of experience using an electronic health record system.

Abilities/Traits:

  • Demonstrate strong clinical knowledge of substance use disorders, integrating DSM 5 and additional criterion to establish treatment recommendations.
  • Provide sound clinical decisions within the parameters of the overall job duties and ensure that clinical recommendations are reflective of the program's mission and values.
  • Must be non-judgmental of clients, develop productive relationships with the diverse client population and have good interpersonal communication and teaching skills with the diverse client population.
  • Must be able to articulate clearly, using good grammar verbally and in writing, as well as ability to make constructive use of professional supervision.
  • Must be able to work independently and organize work effectively using time management tools.
  • Possess knowledge of local community resources and network with agencies to ensure appropriate referrals as needed for client continuity of care.
  • Ability to express self clearly and effectively, orally and in writing.
  • Requires ability to work within a team approach.
  • Requires proficiency with computer skills, programs and email.

Working Conditions:

  • Work will be performed in the Remote setting(s).
  • May require varied work hours depending on the need of clients.
  • Out-of-region travel may be required to attend trainings.
  • May require travel to various locations within Brazoria and Galveston Counties. Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

Special Projects:

  • Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
