The COPSD Specialist will provide services to assist and support the client in developing skills to gain access to services that provide medical, social, educational and other services essential to meeting basic human needs and assist with coordination of individuals with active psychiatric and substance use disorders who require crisis resolution and/or specialized support in treatment due to non-responsiveness with substance use or mental health services resulting from symptomatic worsening and/or disruptive behavior. The LPHA COPSD Specialist will work with both the Substance Use and Mental Health programs to ensure that clients are receiving integrated treatment and connecting to resources within the community to promote stability and recovery.  The COPSD Specialist will maintain a caseload of clients inclusive of creating person-centered recovery plans and be skilled to collaborate with clients to develop and utilize therapeutic interventions based on the client's presenting problems and symptoms. May be asked to conduct diagnostic evaluations for adults as a member of the Mental Health Adult Admissions team (as needed). Will provide care coordination to individuals assigned based on eligibility as determined by the Health Quality Indicator (HQI).

The COPSD Specialist, LPHA will be responsible for the following:

Essential Job Duties:

  • Works collaboratively with the COPSD staff in order to identify needs within the community. 
  • Coordinates with the COPSD staff as well as the Program Assistant to identify needs such as transportation, or medication follow up and provides support to ensure needs are met. 
  • Identifies barriers to service and takes measures to appropriately overcome them. 
  • Monitors client progress in relation to treatment plan. 
  • Utilizes clinically sound judgment in developing relationships with client and service providers. 
  • All documentation is accurate, complete and clear as required by the Center and/or State's procedures; progress notes are completed within specified time frames. Documentation will be in either Smart care and/or CMBHS.
  • Participates in staffing sessions with both Substance Use Program and Mental Health as a member of the clinical team. 
  • Conducts community education programs on substance use treatment and services offered by The Gulf Coast Center Substance Use Program as needed. 
  • Through a system of care, helps to establish baseline data & assists with tracking vital health indicators as a part of CCBHC & 1115 Waiver outcome measures.
  • Must be able to work independently and organize work effectively using time management tools.
  • Conduct drug screenings as needed.
  • Coordinates with Patient Services Specialist for completion of financial screenings.
  • Coordinates with Eligibility Advisor for completion of benefits screening & Medicaid application where applicable
  • Develops person-centered recovery plans based on patient's needs and preferences.
  • Presents patient cases to prescribers.
  • Makes appropriate referrals to community and Center services.
  • Provides therapeutic management strategies and clinical coordination to individuals, groups, and family members as defined by GCC using evidence based strategies within the TRR model of care. 
  • Complete Cognitive Behavioral Therapy (CBT) training and achieve competency as outlined in the current HHSC performance contract. 
  • Will complete LPHA assessment necessary to determine eligibility for admission (inclusive of priority population). 
  • Complete the required diagnosis form indicating all five diagnostic categories.
  • Accurately complete all LPHA Center required documentation necessary for admission process.
  • Assure all required LPHA documentation is printed, signed, and filed in respective record.
  • Conducts service delivery in accordance with Substance Use and Mental Health Adult (MHA) standards as defined by state and local authority.  Assures services are not provided on assigned adults should the Recovery plan and/or ANSA not be valid
  • Conducts the Adult Needs & Strengths Assessment (ANSA) every 6 months or as clinically indicated for individuals as required & assigned. 
  • Responsible for ensuring completion of the initial & annual ANSA recertification in accordance with the Praed Foundation & HHSC performance contract requirements. A lapse/expiration of certification will result in termination of employment.  Exception:  If certification lapse/expiration is reported to supervisor within 24 hours of expiration date, employee will be granted opportunity to recertify by close of following business day.  Any lapse/expiration greater than 24 hours will result in termination of employment. Additionally, all certified ANSA users are required to participate in semi-annual quality assurance training activity conducted by identified ANSA/CANS Super User. 
  • Identifies, arranges, advocates and collaborates with other agencies to assure an effective planned referral, discharge or transfer of services.  (Includes completion of service summary.). 
  • Serve as member of the interdisciplinary treatment team providing individual/group therapy and crisis intervention services as needed and authorized by valid ANSA and Recovery Plan.  Provide crisis services to adults and walk-ins as needed.
  • Display professionalism and team membership when representing program throughout daily work.
  • Participate in team meetings as requested to provide clinical continuity.  Consistently practice provision of follow up and communication related to all services provided.  This is inclusive of other agencies, families and team.
  • Utilize Smartcare and CMBHS Reports to consistently monitor and complete required documentation as indicated by program standards. 
  • Meets or exceeds the established performance requirement for entire caseload each month.  Meets all productivity measures determined by leadership staff.
  • Participate in team meetings as requested to provide clinical continuity.  Consistently practice provision of follow up and communication related to all services provided.  This is inclusive of other agencies, families and team.
  • Effectively communicate with assigned team, collaborative agency members and leadership staff regarding service delivery, continuity of care and employment/administrative issues. 
  • Collaborates with internal program staff (i.e.: case managers, substance use providers, IDD crisis team, etc.) and external community partners (i.e.: primary care physicians, lab, etc.) to coordinate care for individuals and their families. 

  • Completes logistical activities for individuals enrolled in care coordination including tracking of referrals and transitions, addressing barriers to referrals, following up on missed appointments, engaging MCO Service Coordinators as needed and building relationships with a broad range of specialists and providers. 
  • Consistent review of data within Smartcare, CMBHS, and MBOW to monitor health quality indicators ensuring collaboration with interdisciplinary team closing the gap on identified referral needs (i.e. unable to access medication through insurance plan, missing records, identification of an in-network specialist, etc.).
  • Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position.
  • Treat all individuals with the utmost courtesy, decency and respect.

  • Interact with staff, consumers, local entities and advocacy groups to provide a service that meets local needs.
  • Provide highest level of Customer Service for both internal and external customers.
  • Required to report personnel leave; flex time and emergency leave to assigned supervisor.

  • May be required to travel within Brazoria and Galveston Counties.
  • Out of region travel may be required to attend trainings.
  • Must maintain a clear driving record and driver's license in accordance with our Driver Policy.
  • Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.

Required Education:

  • This position requires all candidates to have an LPHA, which includes having a minimum of a master's degree with the following licensures: LCSW, LPC, LMFT.

  • One to two years related Substance Use and/or Mental Health experience is preferred.


  • Demonstrate competence in crisis prevention and management and knowledge of substance use and treatment procedures
  • Ability to tailor COPSD services with Stages of Change applications
  • Detailed knowledge of DSM-V diagnostic criteria for major psychiatric and substance use disorders and the relationship between them
  • Basic knowledge of pharmacology as it relates to individuals with co-occurring disorders
  • Basic knowledge of mental illness and addiction
  • Basic understanding of motivational interviewing techniques
  • Basic knowledge of Trauma Informed Care model
  • Self-motivated with good time management skills
  • Good interpersonal skills
  • Ability to establish cooperative relationships with individuals and their families
  • Mature problem solving approach to daily work
  • Ability to accept responsibility and exercise independent judgement in making decisions
  • Preferred but not required: Knowledge of CMBHS; and experience documenting in an electronic health record.

Working Conditions:

  • May require travel to various locations within Brazoria and Galveston Counties. Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

Special Projects:

  • Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
