Title of Position: Care Coordinator Liaison, QMHP

Description of Duties and Responsibilities: Provide Care Coordination for consumers of Gulf Coast Region Assisted Outpatient Treatment Team. (GCR AOT). Responsible for assisting with implementing a multi-disciplinary AOT program, working with courts, community partners, and other entities to support community based-treatment for adults with SMI who meet criteria for AOT. Collaborate between core partners, identifying and enrolling participants, and developing treatment plans, coordinating admission to program and treatment options for court-ordered treatment. Interface with hospital and court systems to complete outpatient commitment documents. Will be conducting activities that are designed to ensure timely access to needed services and supports, facilitate communication among care providers, eliminate duplication of service, ensure uninterrupted service during transitions of care and track progress toward achievement of treatment goals. Address potential gaps in meeting persons served interrelated medical, social, developmental, behavioral, educational, informal support system, and financial needs in order to achieve optimal health and wellness outcomes, according to preferences of the individual served, as well as applicable Legally Authorized Representative (LAR). Additionally, to coordinate and implement Texas Resiliency and Recovery (TRR) services as part of an interdisciplinary team. This model incorporates resiliency and recovery-oriented services, supports, practices and beliefs into publicly funded mental health delivery systems. As part of an interdisciplinary team, and within the scope of the team, the CCL staff will be responsible for consultation with other team members; developing full range of options in the community settings; rehabilitation self-management and wellness strategies; coordinating medical documentation within medication checkups for GCC consumers, and technical assistance as part of the team in an individual and/or group capacity. The CCL will be responsible for completion of Adult Needs & Strength Assessments (ANSA), Person Centered Recovery Plans (PCRP), to individuals presenting for behavioral health services, brief interventions such as crisis resolution, skills training and education, case management, and liaison between other programs and organizations.

Qualifications for position (minimum):

  • Requires a baccalaureate degree or equivalent in the behavioral sciences field of study from an accredited college or university and/or specialized QMHP certification.

Supervisory Relationships: Receives direct supervision from Project Director-AOT.

Skills & Knowledge Required: The candidate should have general knowledge of mental disorders, be able to establish cooperative relationships with individuals seeking services, professionals, and organizations, and be skilled at working within a team.

Travel & Special Conditions: May be required to travel between Gulf Coast Center Mental Health Clinics and meetings.

Abilities/Traits:

General knowledge of mental health disorders and navigating Electronic Health Records.

Excellent verbal and written communication skills with knowledge to teach and engage in basic problem-solving strategies to support self-directed recovery.

Knowledgeable in trauma informed care and community resources.

Ability to express self clearly and effectively, orally and in writing.

Requires ability to work within a team approach.

Requires proficiency with computer skills, programs, and email.

Working Conditions:

Work will be performed in the Office, Field and/or Remote setting(s).

May require varied work hours depending on need of individuals.

Out of region travel may be required to attend trainings.

May require travel to various locations within Brazoria and Galveston Counties.

Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

Special Projects:

Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
