Job Title:  Population Health Specialist

Location: Remote serving Brazoria and Galveston Counties

Schedule: Part time, 20 hours/week. 4-5 hours a day, based on availability.

Working Status: Unpaid internship - no benefits

Summary:

This position is responsible for ensuring data is accurate, disseminating reports throughout the organization, and management of population health tools. Analyze clinical data to improve quality and cost of population health and other aspects of risk-oriented behavioral healthcare provider operations. The position is responsible for interpretation of the data and identification of appropriate strategies and interventions. This position focuses on quality improvement and evaluation of grant projects in the behavioral health care space. The Population Health Specialist assists in organization-wide efforts regarding data management and reporting.

Job Duties: (To be performed with or without accommodation as based on ADA requirements)

The Population Health Specialist will be responsible for the following:

Essential Job Duties:

  • Coordinates with CCBHC Director regarding data reporting, including data pulls from queries specific to organizational operations, grant requirements and special projects.
  • Develops reports to assist programs, grants, the Project Management Office, and committees in CQI processes.
  • Ensures required data is collected on a timely basis, regularly reports on progress are distributed, and recommendations for future improvements based on the data are accurate.
  • Performs ad-hoc reports as needed.
  • Works with Business Technology and Transformation department to ensure data integrity.
  • Performing chart reviews to extract data for necessary reporting.
  • Works with various data reporting and population health tools
  • Filters, cleans data, and validates reports by reviewing data and performance indicators to locate and correct reporting problems.
  • Extracts and analyzes data to support the organization's decision-making processes.
  • Provides recommendations to the QI Director based on professional expertise, data analytics, and insights regarding the improvement of care and organizational systems.
  • Assists CCBHC Director with meetings, planning sessions, and visualization tools.
  • Treat all individuals with the utmost courtesy, decency and respect.
  • Interact with staff, individuals served, local entities and advocacy groups to provide data that meets local needs.
  • Provide the highest level of Customer Service for both internal and external customers.
  • Required to report personnel leave; flex time and emergency leave to assigned supervisor.
  • Strict adherence to the professional, ethical, and legal requirements set forth in GCC's Business Code of Conduct as well as the Corporate Compliance Plan.
  • Consistent adherence to the Center's Dress Code.
  • Consistent adherence to Center safety guidelines and expectations.
  • May be required to travel within Brazoria and Galveston Counties.
  • Out of region travel may be required to attend trainings.
  • Must maintain a clear driving record and driver's license in accordance with our Driver Policy.
  • Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.

Supervision:

  • Receives direct supervision from CCBHC Director

Required Education:

  • Bachelor's Degree in Population Health, Public Health, Informatics or Data Science

Preferred Education:

ยท  Master's Degree preferred (in Population Health, Public Health or related field).

Required Experience:

  • Minimum of 1 year experience using an electronic health record system.
  • Minimum of 1 year experience with Structed Query language (SQL)
  • Familiarity with CMBHS and SmartCare

Preferred Experience:

  • Experience in an operations role in a healthcare setting preferred.
  • 2 years quality, clinical or medical record experience preferred.
  • Direct care experience
  • 3 years experience using an electronic health record system.
  • 3 years experience with Structed Query language (SQL)

Abilities/Traits:

  • Must be highly proficient in Microsoft Suite and report writing tools.
  • Knowledge of Continuous Quality Improvement practices.
  • Must be non-judgmental of individuals served, develop productive relationships with the diverse client population and have good interpersonal communication and teaching skills with the diverse client population.
  • Must be able to articulate clearly, using good grammar verbally and in writing, as well as ability to make constructive use of professional supervision. 
  • Must be able to work independently and organize work effectively using time management tools. 
  • Possess knowledge of local community resources and network with agencies to ensure appropriate referrals as needed for client continuity of care. 
  • Ability to express self clearly and effectively, orally and in writing.
  • Requires ability to work within a team approach.
  • Requires proficiency with computer skills, programs, and email.

Working Conditions:

  • Work will be performed in the Office, Community, and/or Remotely setting(s).
  • May require varied work hours depending on the need of clients.
  • Out-of-region travel may be required to attend trainings.
  • May require travel to various locations within Brazoria and Galveston Counties. Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

Special Projects:

  • Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
