Job Title: Nurse - RN

Reporting to the Chief Nursing Officer (CNO) & onsite Program Manager, the Behavioral Health registered nurse performs direct patient care & healthcare administrative work within outpatient clinic setting. Duties include coordination of medication clinics and follow-up of adult population diagnosed with severe & persistent mental illness receiving complex pharmacological management and psychiatric diagnostic evaluation services. Provides patient education & guidance about medication and possible side effects. Additional duties include provision of other medication-related services & psychosocial rehabilitative supports to facilitate recovery as outlined in the Texas Resiliency & Recovery service array. As a part of care coordination, provides follow-up to prescriber orders, including referrals to internal/external provider(s) as clinically appropriate. As an RN and trained Qualified Mental Health Professional, may assist with the provision of crisis intervention services when clinically warranted, including safety monitoring when determined necessary. In partnership with treatment team members, is responsible for ensuring quarterly & semi-annual achievement of identified HHSC performance contract measures with goal of improving patient health outcomes.

Job Duties: (To be performed with or without accommodation as based on ADA requirements)
The Nurse - RN will be responsible for the following:
Essential Job Duties:
Serve as patient advocate and navigator to facilitate healthcare access to persons served.
Participate in Patient-Centered Medical Home team meetings.
Facilitate health and disease patient education, including COVID precautions.
Support patient self-management of disease and behavior modification interventions.
Engage in care coordination activities supporting transitions of care; build continuity.
Coordinate continuity of patient care with patients and families following hospital admission, discharge, and ER visits.
Manages high risk patient care, including management of patients with multiple co-morbidities or high risk for a higher level of care setting.
Facilitates patient medication management based upon standing orders and protocols.
Monitor medication adherence through pharmacy data & follow-up with persons served as determined; identify barriers to medication adherence.
Establishes baseline data and tracks vital health indicators gathered at regularly scheduled psychiatric outpatient visits. Support continuous quality improvement of clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
Responsible for the collection and demonstration of key data measures.
Support completion of SAMHSA funding grant initiatives such as engaging persons served to work with identified staff to compete NOMS related data collection.
Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position.

Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.
Non-Essential Job Duties:
1 year in a Patient-Centered Medical Home clinical setting or knowledge of the Patient-Centered Medical Home and/or other primary care setting.
2-5 years' experience in chronic disease management, case management, utilization management, and adult acute care.
