GULF COAST CENTER - JOB DESCRIPTION

Job Title: Case Manager / QMHP - PATH 

Location: Brazoria and Galveston Counties

FLSA Status: Non-Exempt

Working Status: Full Time with Benefits

Summary:

To provide case management to persons who are literally homeless or at risk of homelessness with a diagnosis of mental illness or who have a dual diagnosis of mental illness and substance use in Galveston & Brazoria counties. This employee will serve as liaison with community based homeless services agencies, Gulf Coast Center staff, and other community agencies to provide benefits  and services to consumers with a goal to transition homeless persons  into mainstream mental health  services, provide referrals to mainstream primary health care, substance abuse services, and other resources.

The position is responsible for assessment of homeless individuals and families referred to the Coordinated Entry System to match them to the appropriate housing intervention and community resources to obtain housing stability. The position will screen clients using an assessment workflow through the Homeless Management Information System (HMIS) and will accompany clients from the housing referral through the lease-up process when necessary. The Case Manager will also collaborate with landlords and community partners. The successful candidate will work independently at times and as part of a team at other times; it will also meet regularly with system partners to ensure clients secure housing within applicable program policies, regulations and timelines.

Job Duties: (To be performed with or without accommodation as based on ADA requirements)

The Case Manager / QMHP - PATH will be responsible for the following:

Essential Job Duties:

  • Screens homeless persons for housing recommendations using the Coordinated Entry assessment tool. Conduct immediate intake and assessments for homeless individuals during street outreach/inreach to determine PATH eligibility, mental health connection, connection to resources that may include housing referrals, and other supportive service needs. Provide referrals as applicable to alternate programs if the applicant is not eligible for internal programs and provide resources as necessary.
  • Provides client referral to the appropriate housing intervention and potential housing sites through the Coordinated Entry System and local Coordinated Entry Planning Entities
  • Pulls housing referral from HMIS and begins the next step process to enrollment; Participates in case conferences internally and through local Coordinated Entry Planning Entities
  • Maintain contact with all PATH program participants on an ongoing basis and document interactions in adherence to Gulf Coast Center's policy. Provide assistance to locate mainstream and community resources in obtaining benefits, birth certificates, social security cards and other important documents necessary for housing and supportive services. Assist program participants in gathering documentation necessary and compile social security applications in accordance with the SOAR initiative.
  • Transfers all required homeless and housing documentation to appropriate program staff 
  • Ensures all data is collected and entered accurately and in a timely manner into HMIS
  • Attends Coordinated Entry Assessor/ Housing Navigator, HMIS and other applicable trainings, as assigned, in order to fully utilize the Coordinated Entry system accurately and implement the navigation portion of the program effectively.
  • Provides feed-back to Management Staff and Coordinated Entry Planning Entities regarding Policies and Procedures of the Coordinated Entry system;
  • Provide linkage and support to program participants as they pursue mental health services and mainstream services. Complete referral documentation necessary to connect PATH participants to external mental health and healthcare providers
  • Assist in efforts to connect to mental health services and supportive services for PATH program participant by utilizing existing resources within Brazoria and Galveston Counties
  • Provide consultation to consumers and supportive service providers to develop mental health or overall goals and interventions in the person-centered planning process.
  • Collect data to provide information in HMIS regarding homeless persons and outcomes of program services
  • Provide PATH related trainings to community partners and members
  • Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position.

  • Interact with staff, consumers, local entities and advocacy groups to provide a service that meets local needs.
  • Provide highest level of Customer Service for both internal and external customers.

  • Out of region travel may be required to attend trainings.
  • Must maintain a clear driving record and driver's license in accordance with our Driver Policy.
  • Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.

Non-Essential Job Duties:

  • Assist in development of program informational materials
  • Provide outreach services during annual Point In Time Count for Brazoria and Galveston Counties
  • Attend local resource meetings regularly and maintain accurate resource database. Attend internally meetings as scheduled
  • Collect, organize, pack and distribute hygiene/resource bags
  • Update PATH outreach/contact log weekly
  • Support adult mental health clinics within mental health assessments and intakes as needed
  • Support disaster resource coordination for PATH eligible individuals

Supervision:

  • Receives direct supervision from Program Manager- Coordinated Entry & Supportive Housing

Required Education:

  • Bachelor's degree in Behavioral Science or related field

Required Experience:

  • Experience in shelters or with homeless programs designed for adults with serious mental illness and/or mental illness and substance use disorders; experience in community based mental health programs.

Working Conditions:

  • Work will be performed in the Office, Field and/or Remotely setting(s).
  • May require varied work hours depending on need of clients.
  • Out of region travel may be required to attend trainings.
  • May require travel to various locations within Brazoria and Galveston Counties.
  • Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

Special Projects:

  • Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
